1. Observe proper uniform and haircut for June 1 (Orientation) and June 2 (Enrollment).
2. Take note of the schedules for the UPCAT Orientation. Please attend at your respective schedules so as to avoid any problems with the distribution of the UPCAT Application Forms, too.
3. Students are required to attend the orientation at 1:00 pm. Location is TBA. Parents are highly encouraged to join both orientations, though not required.
4. Students who have not claimed their report cards may claim it at the registrar's office as long as he has his Clearance Form-A and he doesn't get it during the enrollment day.
5. If you are not able to enroll this Thursday, or if you haven't claimed your card which is a requirement to be enrolled, you may enroll on June 6. However, this means that you won't have your ID on that day (incomplete uniform), and this will have its consequences. (-SA)
6. You must provide yourself with the PSHS ID Holder (blue for 4th years) or you won't be able to get your ID. The ID Holder is available at the PSHS COOP for P35.

8. The required ID pictures for the Guidance Office must be placed in a coin envelope. The name of the student should be placed at the upper-left hand corner of the front side. The name of the student should be written like so: LAST NAME, Given Name/s M.I. This envelope should contain the 15 pcs of 1x1, 4 pcs of 1.5x1.5 and 4 pcs of 2x2.
9. The CAT will be requiring each student to provide 1 1x1 ID pictures with a red background. The background of the other required ID pictures were not specified. Please also note that the requirements for CAT will not be collected during the enrollment. The picture will be due on June 10.
10. The collection of the payment for the yearbook will start on June 6, 2011. There will be an official letter emailed to the parents and given to the students regarding the payment and collection within the week.
11. The UPCAT Application Forms will be passed to the Guidance Office by June 10, 2011. In the application form, you'll be required to choose 2 UP Campuses with 2 courses each in order of your preference. Because of our contract with Pisay, we are only allowed to take Science and Technology Courses. The list of the courses which you are allowed to take is available here. Please think about what your choice will be so as to avoid any delay in the filling up of the forms.
